ABSTRACT BACKGROUND AND OBJECTIVES: Psychological intervention can contribute to repairing the quality of life and altering behavior while dealing with chronic orofacial pain, especially those that can become refractory to treatments. Thus, the objective of this study was to present the narratives of three patients with refractory chronic neuropathic pain during the process of psychotherapeutic intervention and the follow-up results. CASE REPORTS: Pain behavior, pain perception and depressive and anxious symptoms were assessed. Patients were under treatment at the Orofacial Pain Clinic and the instruments used in the beginning and end of psychotherapy were the visual analog scale, Beck Inventories (anxiety and depression), Wisconsin Pain Inventory, McGill Pain Questionnaire and Pain Catastrophizing Scale. They attended weekly 50-minute sessions. Each narrative was qualitatively analyzed and the comparison between the evaluations made before and after psychotherapy was included in the context of a phenomenological approach. CONCLUSION: Emotional familiar conflicts and fear of pain crises were the most important aspects described by these patients. Case 1 presented the less cooperative profile and secondary gains. Despite of that, all cases presented improvement and psychotherapy helped to cope with their problems and pain. There was a remarkable impact on the life of patients as well as on their cooperation with pain treatments while helping the patients to build a proactive attitude and to understand their role in their condition.


RESUMO JUSTIFICATIVA E OBJETIVOS: A intervenção psicoterapêutica é uma ferramenta importante para melhorar a qualidade de vida de pacientes com dor orofacial crônica, especialmente aqueles que se tornam refratários aos tratamentos. Assim, o objetivo deste estudo foi apresentar, na forma de narrativa, três casos de pacientes com dor facial refratária ao longo do processo de intervenção psicoterapêutica e os resultados obtidos. RELATO DOS CASOS: Observou-se o comportamento, a percepção da dor e sintomas ansiosos e depressivos. Os pacientes estavam sob tratamento na Equipe de Dor Orofacial e os instrumentos utilizados no início e no final da intervenção foram a escala visual analógica, os Inventários de Beck para Ansiedade e Depressão, o Inventário de Dor de Wisconsin, o Questionário de Dor McGill e a Escala de Catastrofização de Dor. Os pacientes foram atendidos semanalmente em sessões de 50 minutos. Cada narrativa foi analisada qualitativamente e a comparação entre as avaliações (antes e depois do tratamento) foi incluída em um contexto de abordagem fenomenológica. CONCLUSÃO: Conflitos familiares emocionais e medo das crises de dor foram os aspectos mais importantes descritos pelos pacientes. O caso 1 apresentou um perfil menos cooperador, embora todos tenham apresentado melhora em algum âmbito avaliado. A proposta psicoterapêutica os auxiliou no enfrentamento do sofrimento por conta de seus problemas e da dor. Houve um impacto notável na vida dos pacientes como em sua cooperação com os tratamentos, uma vez que aprender a lidar com seu sofrimento os conduziu a uma postura mais proativa através do entendimento de seus papéis no tratamento da condição dolorosa.

ABSTRACT Objective: To describe the first COVID-19 pandemic at Casa Ondina Lobo, a philanthropic nursing home in São Paulo city, and the containment measures against the pandemic that proved to be effective. Methods: Several preventive measures were taken before and during the pandemic, with emphasis on universal testing by reverse transcription polymerase chain reaction for COVID-19. All residents and employees were tested twice in a D9 period. Results: Among the 62 residents and 55 employees, in both testing, eight residents and nine employees tested positive for COVID-19. Of 22% of employees and 75% of residents evolved asymptomatic, emphasizing the importance of universal testing for the detection and isolation of these cases. A quarter of residents evolved without any symptoms, however, with COVID-19 signs, reinforcing the importance of monitoring vital signs. The second testing did not detect any new cases among residents, demonstrating the effectiveness of the containment measures, however, it found four new cases among employees. This emphasized their role in COVID-19 outbreaks in nursing homes. Only one patient died, a 12.5% lethality among those known to be infected and a 1.6% mortality in the total population of residents were seen. Conclusion: The adoption of appropriate containment measures enabled to contain an COVID-19 pandemic in studied nursing home. Universal reverse transcription polymerase chain reaction testing for COVID-19 has proved to be particularly important and effective.

ABSTRACT

Em junho de 2016, a Associação Beneficente "A Mão Branca" de Amparo aos Idosos inaugurou um Centro-dia com a finalidade de atender idosos parcialmente dependentes. Este artigo tem o objetivo de apresentar indicadores relativos aos idosos beneficiários desse serviço, de junho de 2016 a setembro de 2017. Passaram pelo Centro 25 idosos com idade média de 84,3 anos, dois quais 67% são do sexo feminino. A principal procura pelo serviço foi pelos familiares, sendo que os motivos que os levaram a buscar o Centro-dia foram a ociosidade e a redução do convívio social com 72% dos casos, seguidos de 65% dos que apresentaram queixas relacionadas à memória. Fazemse necessários novos estudos voltados a esta nova modalidade do cuidar.


In June 2016, the Associação Beneficente "A Mão Branca" de Amparo aos Idosos has inaugurated a Day Care aiming the partially dependent elderly. This article has the object goal to present the indicators related to the elderly beneficiaries of this servisse from June 2016 to September 2017. Twenty-four seniors 84 years old on average, were enrolled in the service, 67% of them were female. The main demand for the service comes from the elderly 's family, whose concerns about their senior relatives idleness and lack of social interaction, urge them to look for our support expertise. Complaints related to the elderly memory are also strong reasons for the increase of our service demand. Further studies are necessary on this new modality of caring.


En junio de 2016, la Asociación Beneficente "La Mano Blanca" de Amparo a los Ancianos inauguró un Centro-día con la finalidad de atender a ancianos parcialmente dependientes. Este artículo tiene el objetivo de presentar los indicadores relativos a los ancianos beneficiários de esse sevicio, de junho de 2016 a septiembre de 2017. Pasaron por el servicio a 25 ancianos con edad promedio de 84,3 años, dos que el 67% son del sexo femenino. La principal demanda por el servicio fue por los familiares, siendo que los motivos que los llevaron a buscar el Centro-día fueron la ociosidad y la reducción de la convivencia social con el 72% de los casos, seguidos del 65% de los que presentaron quejas relacionadas a la memoria. Se hacen necesarios nuevos estudios orientados a esta nueva modalidad del cuidar.
